Welfare Form

LAL requires parents or guardians of students under the age of 18 years on any of our courses to complete our Welfare Form so that we can ensure the well-being of all our students. It is essential that we know about anything which has a bearing on the health of our students, as well as other information in the event of a problem.

We would like the information to be submitted no later than two weeks before the arrival of the student so that we can make any arrangements for dietary requirements, as well as anything else which may have an effect on the school, such as previous illnesses or injuries.

The form is currently available in a number of different languages. If you need any help, please contact [email protected] or use our live chat service.

We thank you for your co-operation.
